This is not a healthy relationship in any fashion. What if you just tell her you are going to leave her if she doesn't take care of her health? You might get her to think about her priorities.

if she doesn't take her insulin she may go blind and or have multiple amputations she will definitley have serious complications in her future. does she know the consequences.
